Usual and Customary Fee: $125.00 per 50 minute hour.

Insurance & Private Pay
I am currently on the following insurance panels:
- All Regence Blue Cross Blue Shield Plans
- Aetna Health Plans
- MODA Health Plans
- PacificSource Health Plans
If you are covered under one of these plans, I will bill your insurance directly and you will be responsible for your deductible and/or co-pay at the time of your visit. I do not bill secondary insurance.
For other insurance panels, you will need to call the number on the back of your insurance card to find out your mental health benefit coverage. Most insurance plans have some out-of-network coverage which will pay for a portion of your session fees. You may be subject to a deductible or simply have a higher co-pay. It is usually worth your while to check on what your out-of-network benefits are. When you call in this situation ask the following questions:
- Is Jennifer Lopas, LCSW, a covered provider for me?
- What are my out-of-network benefits for mental health?
- Do I have a separate out-of network deductible?
- What percentage or fee do I pay? Is this based on a “usual and customary fee” or her actual rates?
- How many visits do I get in a certain time frame?
Insurance no longer covers charges for services rendered by phone, prior authorization calls or forms, letters, reports, cancellations (if less than 48 hours notice) or missed appointments. These will be billed to you directly. Payment is expected at the time of service. If you do not have a co-pay with you at the time of an appointment, a $10 charge will be added to your bill. Appointments cancelled with under 24-hour notice are billed for the full fee for the scheduled appointment. Secondary insurance is your responsibility. I can provide a form for you to bill your secondary insurance. If you have any questions, please discuss this information with me at any time to determine how it applies to your particular account or insurance policy.

- $170 Evaluation/Assessment (50 minutes)
- $125 Individual Psychotherapy Sessions (50 minutes)
- $200 Couples Evaluation/Assessment (50 minutes)
- $150 Couples Psychotherapy Sessions (50 minutes)
- $125/$150 Late cancellations is the cost of the designated session
NSF/returned checks $25
